What a BBL involves
Buttock fat grafting combines two stages in one operation. First, liposuction removes fat from areas where it is unwanted, commonly the abdomen, flanks, back or thighs, which also refines the waist. The fat is then prepared and injected into the buttocks to improve projection and shape. In keeping with BAAPS safety guidance, the fat is placed only beneath the skin, in the subcutaneous layer, and never into the gluteal muscle. The operation is performed under general anaesthetic and is not a substitute for weight loss.

A typical recovery
- First two weeks: bruising, swelling and soreness, especially in the donor areas; avoid sitting directly on the buttocks and sleep on your front or side.
- Weeks 2 to 6: continue wearing the compression garment; a return to desk work is often possible after about two weeks, using a suitable cushion when sitting is permitted.
- Six weeks onwards: a gradual return to exercise, with the final shape usually judged at around six months once swelling has settled and the surviving fat has established.
Warning signs
Contact the surgical team the same day if you develop a fever, spreading redness or discharge from an incision. Sudden one-sided leg swelling or calf pain needs same-day assessment by a GP or NHS 111. Chest pain or breathlessness needs a 999 call. Fat grafting to the buttocks carries a rare but serious risk of fat entering the blood vessels, which is why these symptoms must never be ignored.
