The key difference is what each procedure treats. Thigh liposuction removes stubborn fat through tiny incisions and relies on your skin tightening afterwards, so it suits people with good skin elasticity. A thigh lift removes loose, sagging skin (and some fat) through longer incisions, so it suits people whose main problem is excess skin, often after weight loss. Some people benefit from both, combined in one operation.

Understanding the thigh

The shape of the thigh depends on the underlying muscle, the amount and distribution of fat, and the quality of the skin. With weight gain, fat often collects on the inner and outer thighs and around the knees. With weight loss, pregnancy or ageing, the skin can lose its elasticity, and once stretched beyond a certain point it cannot spring back. Deciding between liposuction and a thigh lift comes down to which of these factors is causing your concern.

  • Main target: liposuction treats localised fat; a thigh lift treats loose, excess skin.
  • Ideal skin: liposuction needs good elasticity; a thigh lift is designed for skin that has lost it.
  • Scars: liposuction leaves a few small scars a few millimetres long; a thigh lift leaves a groin scar and sometimes a vertical scar down the inner thigh.

  • Cost: liposuction from £7,500 for one area, £8,500 for two and £9,500 for three; Vaser liposuction from £8,000 for one area and £9,000 for two; thigh reduction from £12,000.

When liposuction is the better choice

Liposuction suits adults at or near a healthy, stable weight who have pockets of fat on the inner or outer thighs, or around the knees, that have not responded to diet and exercise, and whose skin is firm. Younger skin generally tightens well after fat removal. Vaser liposuction uses ultrasound energy to loosen fat before removal and may give some additional skin contraction in selected cases. When a thigh lift is the better choice

If the skin hangs in folds, crinkles when you stand, or chafes and causes rashes, liposuction alone may make matters worse by removing the fat that is filling out the skin, leaving it looser. In these situations, removing the excess skin is the most reliable way to improve shape. Combining the two

Many thigh lifts include liposuction. Fat is first removed to refine the contour, and then the loose skin is lifted and excised. This can improve the overall shape more than either procedure alone. Combining them does lengthen the operation and recovery, and the plan must balance the desired contour with the safety of the skin’s blood supply. For milder skin laxity, energy-assisted liposuction such as BodyTite may be considered as a middle option.

Which is right for you?

A simple guide: if you can pinch a thick layer of fat but the skin is smooth and springy, liposuction is likely to suit. If the skin is thin, loose and folds when you stand, a thigh lift is more likely to help. If you have both fat and loose skin, a combined approach may be recommended. Your weight stability, general health, smoking status and feelings about scars also matter. Consultation

Can liposuction tighten loose thigh skin?

Only slightly. Liposuction relies on the skin to contract; if the skin is already loose, it may look looser afterwards.
