Vertical cords running down the front of the neck, and loose skin that hangs below the chin, are among the most common reasons people seek facial rejuvenation. They are often called a ‘turkey neck’. Understanding what causes them explains why some treatments work and others do not.

What are platysmal bands?

The platysma is a thin sheet of muscle that runs from the collarbones up over the jaw. When we are young its two sides meet smoothly in the middle of the neck. With age the inner edges separate and become loose, so they show as two vertical bands, especially when you talk or grimace.

What causes a turkey neck?

  • Ageing: loss of skin elasticity and loosening of the platysma.
  • Fat: fat above or below the muscle blurs the angle between the chin and neck.
  • Weight loss: can leave loose skin behind.
  • Genetics and anatomy: a small chin or low-set voice box can make the neck look fuller at any age.
  • Sun damage and smoking accelerate skin ageing.

Non-surgical options

Anti-wrinkle injections into the bands can soften them temporarily in selected patients with good skin tone, and effects last a few months. Energy-based skin tightening can help mild laxity. Neither treatment removes loose skin.

Liposuction

When the main problem is fat under the chin and the skin is still elastic, neck or double chin liposuction may be enough to restore a sharper jawline.

Neck lift and deep plane face and neck lift

When the skin is loose and the bands are visible, surgery gives the most reliable result. A neck lift tightens the platysma, often through a small incision under the chin, and redrapes the skin through incisions around the ears. A deep plane face and neck lift treats the jowls and neck together by lifting the deeper tissues, which in Mr Nassab’s experience gives a more natural, longer-lasting result than pulling the skin alone.

Recovery

After a neck lift or face and neck lift, most people feel comfortable going out after about two weeks, with swelling settling over several months. Risks include bleeding, nerve injury, scarring and asymmetry, which are discussed in detail at consultation.
