Knee fat usually refers to a soft pad of fat on the inner side of the knee, just above the joint. Its size is largely inherited and it often stays put even when the rest of the leg slims down. Exercise and a healthy weight help overall leg shape, but they cannot target this pad directly. For adults at a stable weight, liposuction of the inner knee can refine the contour, frequently alongside the inner thighs.

Where knee fat sits

The most common site is the inner (medial) knee, where a rounded fullness can blur the line between thigh and calf. Some people also notice fullness just above the kneecap or, less often, on the outer side. From the front, an inner knee pad can make the legs look straight rather than gently tapered, and it often shows most in dresses, shorts and fitted trousers. The skin over the knee is relatively thin, and with age it can also become crepey and lax.

Why it happens

  • Genetics: inherited fat distribution is the main reason some people have a prominent inner knee pad.
  • Hormones: oestrogen encourages fat storage in the lower body, so the pattern is more common in women.
  • Weight gain: additional weight often shows on the thighs and knees.
  • Ageing: thinner skin and a loss of elasticity make any fullness look softer and more noticeable.
  • Fluid: puffiness around the knees and ankles can come from long periods of standing, heat or hormonal changes.

When knee swelling is not fat

A soft fat pad develops slowly, feels the same on both sides and is painless. See your GP if a knee becomes swollen, hot, red or painful, if swelling follows an injury, if there is a firm lump behind the knee, or if the joint locks or gives way, as these point to a joint or soft-tissue problem rather than fat. Sudden swelling of one calf or leg, especially with pain, needs same-day advice from your GP or NHS 111 because it may be a blood clot. What exercise and lifestyle can do

Strength work for the thighs, such as squats, lunges and cycling, improves muscle tone and can make the leg look more shapely. Reducing overall body fat through diet and activity tends to slim the legs in proportion. What exercise cannot do is burn fat from a single chosen spot, so an inherited inner knee pad may remain even in very active people. Non-surgical fat-reduction devices and injections are marketed for this area; results vary, several sessions may be needed, and they are best suited to small amounts of fat.

Knee liposuction

Liposuction of the inner knee is usually performed through one or two tiny incisions placed in a natural skin crease. A fine cannula removes a carefully judged amount of fat, sculpting a smoother transition from thigh to calf. Because the area is small and the skin is thin, precision matters more than volume: taking too much can create a hollow or irregular surface. The knees are often treated with the inner thighs, so that the whole inner leg line flows. Mr Nassab will assess skin quality, as lax skin will not always tighten and may limit the result.

Recovery after knee liposuction

Expect bruising, swelling and tightness around the knees for the first two weeks, with swelling often gravitating towards the ankles. A compression garment, sometimes extending to the ankle, is worn for around six weeks. Walking is encouraged from the first day. Most people return to desk work within a week, and to running or high-impact exercise after about six weeks. The final shape can take three to six months to appear as residual swelling settles. Risks include irregularity, asymmetry, prolonged swelling, numbness and, as with any operation, infection and blood clots.

Cost

Liposuction costs from £7,500 for one area, £8,500 for two areas and £9,500 for three. Inner knees combined with inner thighs are commonly quoted as two areas. The £150 consultation fee is separate. Cosmetic surgery is for adults aged 18 and over.

Can exercise get rid of knee fat?

Exercise improves tone and overall body fat, but it cannot reduce fat in one spot. An inherited inner knee pad often remains.

Is knee liposuction worth doing on its own?

It can be, but many people find the result looks more natural when the inner thighs are contoured at the same time. This is discussed at consultation.

Will knee liposuction leave scars?

The incisions are only a few millimetres long and are usually placed in a skin crease, where they tend to fade well over a year or so.

How long does swelling last after knee liposuction?

Most swelling settles within a few weeks, but mild swelling around the knee and ankle can persist for a few months.
