‘Cankles’ is the informal name for lower legs where the calf seems to merge into the ankle without a defined ankle shape. For most people it is simply a matter of genetics and fat distribution, but swollen ankles can sometimes be a sign of a medical problem, so it is important to know the difference.

What causes cankles?

  • Genetics: the shape of your calves, ankles and bones is largely inherited.
  • Fat distribution: some people store fat around the lower calf and ankle that does not shift with diet and exercise.
  • Fluid retention: ankles can swell with heat, long periods standing or sitting, pregnancy and some medicines.
  • Lipoedema: a condition, almost always in women, in which fat builds up symmetrically in the legs, often sparing the feet, and can be painful and bruise easily.

When to get medical advice

If one leg is suddenly swollen, painful, red or hot, get urgent same-day advice from your GP or NHS 111, as this can be a sign of a blood clot. If you also have breathlessness or chest pain, call 999. New swelling in both legs, or swelling that leaves a dent when pressed, should be checked by your GP, as it can be linked to heart, kidney, liver or vein problems that need medical assessment rather than cosmetic treatment.

Can exercise fix cankles?

Calf-strengthening exercise, weight management and reducing salt can improve overall leg shape and fluid retention, but you cannot target fat loss from one spot, and inherited fat deposits around the ankle often remain.

Calf and ankle liposuction

Where cankles are caused by stubborn fat and the skin is elastic, liposuction can reshape the lower leg and define the ankle. It is delicate work because the area has little fat and important structures close to the surface, so a conservative, even approach is essential. Compression garments are worn for several weeks and swelling in the lower legs can take several months to settle fully.

Lipoedema

If you may have lipoedema, it is important to have it properly assessed. Treatment is different from routine cosmetic liposuction and may involve compression, lifestyle measures and, in selected cases, specialist liposuction.
