A bulbous nose has a rounded, wide or ‘ball-shaped’ tip that can look heavy compared with the rest of the nose. It is one of the most common reasons people ask about rhinoplasty, and it can usually be refined while keeping the nose natural and in balance with the face.
What causes a bulbous tip?
The shape of the tip is set by two pairs of cartilages (the lower lateral cartilages) and the skin that covers them.
- Wide or convex cartilages: cartilages that are broad, curved or set far apart create a round tip.
- Thick skin: thick, oily skin with large pores hides the underlying shape and makes the tip look fuller.
- Weak tip support: can make the tip droop and look heavier.
- Genetics and ethnicity strongly influence tip shape.
A red, enlarged, bumpy nose that develops later in life can be caused by rosacea (rhinophyma), which is a different condition and treated differently.
Can filler fix a bulbous nose?
Filler adds volume. It can straighten a profile or disguise a small bump, but it cannot make a wide tip narrower and should be used with great caution near the nasal tip.
Tip rhinoplasty
In a tip rhinoplasty the cartilages are reshaped using sutures, careful trimming or small cartilage grafts to create a more defined, better-supported tip. This can often be done without breaking the nasal bones. Where the skin is thick, the change is more subtle, and the skin may take longer to settle.
Recovery
Most patients wear a small splint for about a week and return to work after one to two weeks. Tip swelling is the last to settle and can take up to a year, particularly with thicker skin.
