Most people return to desk-based work one to two weeks after breast asymmetry correction, resume driving at around two weeks and return to full exercise at about six weeks. Because the two breasts may have had different procedures, they often swell and settle at different rates, so it can take three to six months, and sometimes up to a year, for the final balance to be clear.

Why recovery can feel uneven

Breast asymmetry correction frequently involves a different operation on each side: an implant on one side and a lift on the other, implants of two sizes, or a reduction of the larger breast. Each procedure heals in its own way. A breast that has had more surgery may be more swollen, sit higher or feel firmer for longer. This temporary imbalance can be unsettling, but it is expected, and judging the result too early often causes unnecessary worry.

The first 48 hours

You will wake with dressings and a supportive post-operative bra. Tightness, aching and tiredness are normal, and regular pain relief helps. Most patients go home the same day or the following morning, with someone to drive them and stay with them overnight. Walk around the house gently every few hours to keep your circulation moving, and sleep propped up on pillows. Drains are not always needed; if one is used, it is usually removed before you go home or within a day or two.

Week one

  • Support: wear your surgical bra day and night unless advised otherwise.
  • Dressings: keep them clean and dry, and follow instructions about showering.
  • Arms: avoid lifting anything heavy, including young children, and reaching above shoulder height.
  • Rest: short walks are encouraged, but plan plenty of rest and help with household tasks.
  • Review: your first wound check is usually around one week after surgery.

Weeks two to six

Swelling and bruising fade steadily. Many people return to desk-based work after one to two weeks; jobs involving lifting or physical activity may need longer. Driving is usually possible from around two weeks, once you can wear a seatbelt comfortably and make an emergency stop. Lower-body exercise and brisk walking can start gradually from about three to four weeks, with upper-body and high-impact exercise, including running and swimming, usually resumed at around six weeks after review.

Three to twelve months

Implants gradually settle into their final position, often described as ‘dropping and fluffing’, over several months. A breast that has been lifted or reduced may also change shape as swelling resolves. Differences in how the two sides settle usually even out during this period. Scars are pink at first and mature gradually over 12 to 18 months. Mr Nassab will see you at planned intervals to check progress.

Sleep, bras and scar care

Sleep on your back, propped up, for the first few weeks; side sleeping is usually comfortable from around four to six weeks. Continue wearing a supportive, non-wired bra for about six weeks before moving to underwired styles when advised. Once wounds have fully healed, silicone gel or sheets and gentle massage can help scars mature, and keeping scars out of the sun for the first year helps prevent darkening.

Warning signs and who to contact

  • Contact the surgical team urgently: if one breast rapidly swells, becomes tense or very painful, which may indicate bleeding (haematoma).
  • Contact the team the same day: for fever, spreading redness, discharge from a wound or a wound that opens.
  • Same-day GP or NHS 111: for swelling or pain in one calf or leg.
  • Call 999: for chest pain or sudden breathlessness.

When the result can be judged

It is sensible to wait at least three to six months before assessing the final balance, as swelling and implant settling continue. Some degree of asymmetry is natural and may remain.
