Abdominal etching, often called six-pack surgery, is a form of high-definition liposuction that removes fat in a precise pattern to reveal or enhance the outline of the abdominal muscles. It works best for people who are already lean and active, with good skin quality and a small layer of stubborn fat hiding muscle definition. It does not build muscle, and results depend on keeping a stable weight.
Why can’t I see my abs?
Everyone has abdominal muscles, and the ‘six-pack’ shape comes from the rectus abdominis muscle divided by bands of tendon. Whether it is visible depends mainly on the thickness of fat over it. Genetics play a large role in where fat is stored, and many fit people keep a layer of fat on the lower abdomen and flanks despite diet and training. Muscle development, skin thickness and, after pregnancy, separation of the abdominal muscles (diastasis recti) also affect how the abdomen looks.
What abdominal etching involves
High-definition liposuction usually uses Vaser (ultrasound-assisted) liposuction, which uses sound energy to loosen fat before it is removed through fine cannulas. The surgeon marks the muscle borders with you standing and contracting your abdomen. Fat is thinned over the muscle bellies and removed more deeply along the natural grooves: the midline, the lines at the sides of the rectus muscle and the horizontal tendon lines. Some fat is left to keep the result looking natural and to avoid a ‘drawn-on’ appearance. The flanks and lower back are often treated too to improve the overall shape.
Who is suitable?
- Lean build: a body weight at or near your healthy target, with a relatively thin layer of fat.
- Stable weight and lifestyle: regular exercise and a diet you can maintain.
- Good skin elasticity: loose skin will not tighten enough after fat removal; a tummy tuck may suit better.
- Realistic expectations: the aim is enhanced definition, not a body-builder physique.
- Adults 18+ in good general health, ideally non-smokers.
Abdominal etching is not a weight-loss treatment and is not suitable where fat lies mainly deep inside the abdomen, around the organs, which liposuction cannot remove.
Non-surgical alternatives
A structured training programme, attention to diet and adequate sleep remain the foundation. Fat-freezing and other energy-based devices can reduce small fat pockets by a modest amount, usually over several sessions, but they are not precise enough to sculpt muscle lines. They may suit people who want a subtle change without surgery.
Recovery
Expect swelling, bruising and firmness, and a compression garment is typically worn for around six weeks, sometimes with foam pads over the etched lines. Many people return to desk-based work after one to two weeks and to light exercise after about four weeks, building up gradually. Swelling can take several months to settle fully, and the definition usually becomes clearer at around three to six months.
Risks and limitations
Risks include bleeding, infection, seroma, blood clots, numbness, skin irregularities, asymmetry, burns from ultrasound energy, and pigmentation changes. A particular risk with etching is an unnatural look, either lines that are too sharp or asymmetrical, or a result that looks odd if you later gain weight, because fat can build up around the etched grooves. Revision surgery is sometimes needed. How much does six-pack surgery cost?
The cost of abdominal etching is quoted at consultation because it depends on the areas treated. For reference, Vaser liposuction is £8,000 for one area and £9,000 for two areas. The consultation fee of £150 is separate.

Is abdominal etching permanent?
Fat cells removed do not come back, but remaining fat can grow with weight gain, which can blur or distort the result. A stable weight is important.
Does six-pack surgery build muscle?
No. It removes fat to reveal the muscles you already have. Training is still needed to maintain muscle tone.

What if I have loose skin?
