Motiva Preservé RecoveryA realistic week-by-week guide to healing after minimally invasive breast augmentation

Medically reviewed: Last reviewed by Mr Reza Nassab, FRCS Plast —

Recovery after Motiva Preservé is often gentler than after a conventional breast augmentation because the incision is short and less tissue is disturbed. Many people are up and about the same day and return to desk work within about a week, but the breasts still need around six weeks before strenuous exercise and several months to settle fully.

Why recovery can feel easier

Preservé uses a short incision, precise pocket creation and minimal disruption of muscle, which is thought to reduce pain and bruising in the early days. Many patients have the operation under local anaesthetic with sedation, so they avoid the grogginess of a general anaesthetic. That said, it is still surgery, and your body needs time to heal internally even when you feel well. Treat the timeline below as a guide rather than a target.

The first 48 hours

Expect tightness, aching and a feeling of pressure across the chest rather than sharp pain. Regular simple pain relief, as advised by the team, is usually enough. You should be walking around the house from the first evening to help circulation. Wear your surgical bra day and night, keep dressings dry and have someone with you for the first night. Avoid lifting anything heavier than a kettle and do not raise your elbows above shoulder height.

Week one

Most people feel noticeably better each day. Swelling and some bruising are normal, and the breasts may look high and firm at first; this is expected as the tissues settle around the implants. Showering is usually allowed after a couple of days according to the dressing advice you are given. Many patients who work at a desk feel ready to return towards the end of the first week, and you will usually have a wound check in the first week or two.

  • Driving: only once you are off strong pain relief and can perform an emergency stop and turn the wheel comfortably; for many this is one to two weeks.
  • Sleep: on your back, propped up on pillows, for the first few weeks.
  • Lifting: avoid heavy shopping, children or pets for several weeks.

Weeks two to six

Energy returns and day-to-day life feels normal for most people, but the internal healing that holds the implant pocket is still maturing. Gentle walking can gradually increase from the start. Light lower-body exercise may be introduced from around three to four weeks if the team agrees. Avoid chest exercises, running, swimming and anything that bounces or strains the chest until at least six weeks. Continue to wear a supportive, non-wired bra as instructed. Jobs involving physical work may require longer off.

Three to twelve months

Over the following months the implants soften and settle, the breasts take on a more natural shape and any remaining swelling resolves. Sensation changes, such as numbness or heightened sensitivity of the nipples, usually improve gradually. Underwired bras can often be worn again after around three months, depending on healing. The final result is usually assessed at around six months to a year.

Caring for your scar

Once the wound has fully healed, gently massaging with a silicone gel or using silicone sheets can help the scar mature. Keep the scar out of the sun or covered with a high-factor sunscreen for at least a year. Scars typically look pink or raised for a few months before fading. The Preservé scars guide explains this in more detail.

Warning signs and who to contact

Mild swelling, bruising and tightness are normal. Contact the team if something feels different or is getting worse.

  • Rapid swelling of one breast: or a breast that becomes very tense and painful: contact the surgical team urgently.
  • Fever or spreading redness: around the wound, or discharge: contact the team the same day.
  • Sudden one-sided leg swelling or calf pain: same-day GP or NHS 111.
  • Chest pain or breathlessness: call 999.

Frequently Asked Questions

How long off work is needed after Preservé?

Many people with desk jobs return after about a week. Physical or lifting-heavy roles usually need longer, often several weeks.

When can exercise restart?

Walking can start straight away. Light lower-body exercise may be possible from three to four weeks, with full exercise including chest work from around six weeks if healing is on track.

Is a special bra needed?

Yes. A supportive surgical bra is worn day and night for the first weeks, then a soft non-wired bra until the team advises otherwise.

When will the breasts look natural?

They often look high and firm at first. Most settling happens over the first three months, with the final shape seen at six to twelve months.

Is recovery faster than traditional augmentation?

Many patients find it gentler, but everyone heals differently, and internal healing still needs the full six weeks.
