A double chin is a fold of fullness beneath the jaw, usually caused by fat under the chin (submental fat), genetics, a small or set-back chin, loose skin or posture. Many slim people have one because of their inherited fat distribution or jaw shape. Where stubborn fat is the main cause and the skin is reasonably elastic, double chin liposuction can improve the jaw and neck contour.
What causes a double chin?
- Submental fat: fat that collects under the chin and is often resistant to diet and exercise.
- Genetics: some families tend to store fat under the chin, regardless of weight.
- Chin and jaw shape: a small or set-back chin, or a low position of the hyoid bone in the neck, reduces definition.
- Skin laxity: with age the skin and neck muscles loosen, producing sagging and neck bands.
- Weight gain: general weight gain often shows early under the chin.
- Posture: looking down at phones and screens temporarily exaggerates a double chin.
When to see a GP
See your GP if you notice a new lump under the jaw or in the neck, especially one that is hard, growing or painful, or if swelling is accompanied by difficulty swallowing, fever, or a change in your voice. A swelling at the front of the neck that moves when you swallow may relate to the thyroid gland and should be checked.
Non-surgical options
A healthy diet, regular exercise and good posture help, although spot reduction of chin fat through exercise alone is not possible. Fat-reducing injections and cooling devices are available from some providers; they may need several sessions, can cause significant temporary swelling, and results vary. Energy-based skin tightening treatments can help mild laxity. Enhancing a small chin with dermal filler can improve the profile and reduce the appearance of a double chin in some people.
Double chin liposuction
Liposuction of the chin and neck uses a fine cannula inserted through tiny incisions, usually under the chin and sometimes behind the earlobes, to remove excess fat. It can often be performed under local anaesthetic. The aim is a clearer angle between the jaw and neck. Skin elasticity is key: in younger patients the skin usually tightens well afterwards, whereas in people with significant loose skin, liposuction alone may leave laxity. Mr Nassab may discuss combining liposuction with FaceTite (radiofrequency skin tightening), or a neck lift where there is marked sagging.
Is liposuction or a neck lift better?
It depends on what is causing the double chin. Mainly fat with good skin quality usually responds to liposuction. Loose skin, neck bands and jowls usually need a neck lift, sometimes with a facelift, to address the underlying tissues. Mr Nassab will examine your skin tone, fat distribution, chin projection and neck muscles to recommend an appropriate approach.
Recovery and risks
You will usually wear a compression garment around the chin and neck, full time at first and then at night, for about one to two weeks. Swelling and bruising are common, and it can take around three months for the final contour to become apparent. Risks include irregularities, asymmetry, prolonged swelling, numbness, infection, and uncommonly injury to the nerve that controls the corner of the mouth, which is usually temporary.
Who is a good candidate?
The best candidates for chin liposuction are adults with a localised pocket of fat under the chin, good skin elasticity and realistic expectations. People with significant loose skin, prominent neck bands or a very small chin may benefit more from other approaches, alone or in combination, and this will be discussed openly.
Cost and next steps
Double chin liposuction is quoted at consultation, as the price depends on whether it is combined with other procedures. FaceTite costs from £7,500. The consultation fee of £150 is separate. Cosmetic surgery is for adults aged 18 and over who are at a stable weight.
Frequently Asked Questions
Why can a slim person have a double chin?
Genetics, a small chin, or the position of the neck structures can create a double chin regardless of body weight.
Can exercises get rid of a double chin?
Chin exercises and posture work may improve muscle tone, but they cannot target the fat itself.
Is chin liposuction permanent?
The fat cells removed do not return, but significant weight gain can still cause fullness under the chin.
Will there be visible scars?
The incisions are only a few millimetres long and usually placed under the chin, where they typically fade well.
