A Brazilian butt lift adds volume and shape using your own fat, while a surgical buttock lift removes loose, sagging skin to lift the buttocks. A BBL suits people with good skin tone who want more projection; a buttock lift suits people, often after major weight loss, whose main problem is excess skin. Some people need elements of both. The choice depends mainly on skin quality, available fat and how much scarring you are prepared to accept.
What a BBL does
A Brazilian butt lift combines liposuction of areas such as the abdomen, flanks and back with transfer of the collected fat into the buttocks. It adds volume and projection and slims the waist, which improves proportion. Following BAAPS safety guidance, fat is placed only in the subcutaneous layer. The incisions are small. A BBL can create a mild lifting effect by adding fullness to the upper buttock, but it does not remove skin, so it cannot correct significant sagging.
What a buttock lift does
A buttock lift is a skin-removal operation. A band of excess skin and fat is removed, usually through an incision across the lower back just above the buttocks, and the remaining skin is tightened to lift the buttocks and outer thighs. It is often performed as part of a lower body lift after major weight loss, sometimes combined with a tummy tuck. It leaves a long scar that is typically positioned to sit within underwear, and on its own it may reduce volume rather than add it. Mr Nassab will advise whether this type of surgery is appropriate for you and, where needed, discuss onward referral.
Side-by-side comparison
- Main purpose: BBL adds volume and shape; a buttock lift removes loose skin and lifts.
- Best suited to: BBL: good skin tone with spare donor fat. Buttock lift: excess, sagging skin, often after weight loss.
- Scars: BBL: several small incisions. Buttock lift: a long scar across the lower back or buttock crease.
- Effect on volume: BBL increases it; a buttock lift alone can flatten the buttocks slightly.
- Recovery: BBL: avoid sitting on the buttocks for a period and wear compression. Buttock lift: care with bending and wound healing for several weeks.
- Specific risks: BBL: uneven fat survival and a rare risk of fat entering blood vessels. Buttock lift: wound breakdown, seroma and scar problems.
Recovery compared
After a BBL, most people return to desk work in about two weeks, with compression garments worn for several weeks and the final result seen at around six months. After a buttock lift, recovery tends to be longer: drains may be used, bending and sitting need care to protect the wound, and it may take several weeks before normal activities resume. Scar maturation for both continues over 12 to 18 months, although the buttock lift scar is much longer.
When both are considered
Some patients have loose skin and also want more volume. In selected cases a buttock lift may be combined with the patient’s own tissue or fat being used to preserve or add volume, and occasionally surgery is staged. This is more complex and needs careful planning around weight stability, skin quality and safety.
Which is right for you?
- A BBL may suit you if: your skin is reasonably firm, you want more projection and curves, and you have spare fat in areas you would like slimmer.
- A buttock lift may suit you if: your buttocks have lost shape after significant weight loss and loose skin hangs or folds.
- Neither may be needed if: your concern is mild; glute training or liposuction of the waist and flanks may be enough.
Weight stability matters for both: aim to be at a steady weight for at least six months before surgery.
Cost and next steps
Buttock fat grafting (BBL) costs from £12,500. A buttock lift is quoted at consultation. The £150 consultation fee is separate. At your appointment Mr Nassab will assess your skin, fat distribution and goals, and explain which option, if any, is likely to help. Cosmetic surgery is for adults aged 18 and over.
Frequently Asked Questions
Does a BBL lift sagging buttocks?
It can give a mild lift by adding volume to the upper buttock, but it does not remove loose skin.
Which leaves more scarring?
A buttock lift leaves a long scar; a BBL leaves several small incision scars.
Is a buttock lift better after weight loss?
If loose skin is the main problem after major weight loss, skin removal is usually more effective than fat grafting alone.
Can a BBL and buttock lift be combined?
Sometimes, in carefully selected cases, but it is more complex and may be staged.
Which operation has the longer recovery?
A buttock lift usually has a longer recovery because of the long wound, while a BBL requires time avoiding direct pressure on the buttocks.
