After 360 liposuction, most people return to desk-based work in around one to two weeks and resume full exercise at four to six weeks. A compression garment is usually worn for about six weeks. Bruising settles over two to three weeks, but swelling around the waist and back takes several months to resolve, and the final contour is typically seen at six months or later.

The first 48 hours

Because 360 liposuction treats the front, sides and back, you will feel sore all the way around your midsection, a little like a deep bruise or after an intense workout. The small incisions may leak blood-tinged fluid for a day or two; this is normal and absorbent pads are placed in the garment. You will be encouraged to walk within hours of surgery to help circulation. Dizziness when standing is possible in the first day, so move slowly and have someone with you. Many patients go home the same day or after one night.

The compression garment

Your compression garment supports the tissues, helps control swelling and encourages the skin to settle smoothly over the new contour. It is typically worn day and night for the first few weeks, removed only for showering, then often during the day or night for a further period, totalling around six weeks. Foam pads beneath the garment may be recommended. Check the skin daily for rubbing or pressure marks, and let the team know if the garment feels too tight or causes numbness in the legs.

Weeks one to six

  • Week one: bruising is at its most visible and movement feels stiff; short, frequent walks and plenty of fluids help recovery.
  • Work: many return to desk-based work at one to two weeks; physical jobs usually need longer.
  • Driving: once you can sit comfortably, twist to check blind spots and brake firmly, often at one to two weeks.
  • Exercise: walking throughout; light gym work from about three to four weeks; full training, including core exercises, from four to six weeks.
  • Swelling: it often increases in the evening and after activity; lower-back and flank swelling may persist longer than at the front.

Months two to six and beyond

The treated areas may feel firm, lumpy or numb for some weeks as the tissues heal. This usually softens gradually, and gentle massage may be recommended once the incisions have healed. Some patients are offered lymphatic drainage massage; this should be discussed with the surgical team first. Clothes often fit differently by around three months, but the final shape continues to refine for six months or more. Maintaining a stable weight through diet and exercise helps preserve the result, as remaining fat cells can still enlarge with weight gain.

Sleep, sitting and daily life

Sleeping on your back is usually most comfortable in the first week or two, although lying on one side may be possible if it feels comfortable, since both the front and back are treated. A pillow under the knees and a supportive mattress help. Sitting for long periods may feel uncomfortable at first where the lower back was treated. Wear loose clothing over your garment, and avoid baths, swimming and hot tubs until the incisions have healed.

Scar care

The incisions are only a few millimetres long and are placed in discreet locations, such as the natural creases of the lower back, flanks and near the belly button. They typically fade to small pale marks over 12 to 18 months. Once healed, silicone gel may help, and protecting them from sun exposure reduces the chance of darkening.

Warning signs

Contact the surgical team the same day for fever, spreading redness around an incision, increasing pain or foul-smelling discharge. A soft, fluid-filled swelling that develops after a week or two may be a seroma and should be checked. Sudden calf pain or one-sided leg swelling needs same-day assessment by your GP or NHS 111.
